It is an add-on that widens the Front Bilateral Suit, giving slightly larger dogs more girth room in the size you already own. It adds circumference, not length.
If your dog fits the length and shape of a size but needs a little more room around the body, the extender lets you keep that size and add girth room, instead of jumping to a full size larger.

The left side of the extender connects to the left half of the suit's zipper and the right side to the right half, widening the suit down the middle. No tools and no sewing.
Width only. It adds girth (circumference) room and does not make the suit longer.
